1. Registered Name: 

 The racing name of the Greyhound

2. Call Name: 

 The name you normally call to get his attention

3. Tatoos: 

 Marks printed on the inside of each ear

4. Sex:

 Male or female
5. DoB - Date of Birth Include the date of birth in the format "yyyy/mm/dd"

6. Colour: 

 Using the American Greyhound Official Colour Chart as a guide, fully describe the colour of the Greyhound to permit easy identification.

7. Weight:

 Weight in pounds (lbs)or kilogram (kgs) when last weighed

8. Demeanor:

Keywords can be combined to get a more accurate identification of the demeanor of the Greyhound

9. Greyhound Description:

Provide a detailed physical description the Greyhound

10. Collar:

Name or describe the type and size of collar normally used, including colour(s) and any unusual clasping mechanism and anything that might be hooked on to the collar

11. Leash:

Name or describe the type and size of leash normally used, including colour(s) and any unusual clasping mechanism

12. Tags:

Indicate what tags are attached to the collar, and provide the information printed on the tags

13. Medical Requirements:

Give a detailed description of all medical conditions currently been experienced by the Greyhound

14. Medications:

List all medications currently being administered, the dosage of each, the time normally given, and the Veterinarian who prescribed them

15. Veterinarian:

Provide the name and address of the Veterinarian or clinic you currently use

16. Owner Information:

Provide full names and address of the adopted owner, including all phone number at which you can be reached

17. Other Contact:

Provide the names and addresses of anyone who would normally look after the Greyhound when you are away
